What is my Subscriber /ID Number?
Your Subscriber / ID Number is generally either your Social Security Number, or an alternate ID number assigned by your employer. If your employer has assigned an alternate ID number, it is printed on the ID Card that we mailed to you.
How can I print an ID card?
You may print an ID card once you log in, but you do not need one to visit your dentist. Your provider will be able to find your benefit information using our automated suite of tools. If you still would like an ID card, please log into your account and print one to take to your appointment.

What kind of plan do I have and what are my benefits?
You can easily determine what plan you have and the services it covers by logging into your account. Just enter your username and password in the Subscriber Login box. What dentists can I see?
Your coverage determines the providers you may see, but Delta Dental’s got you covered with the most in-network providers in the state. To find out if a dentist participates in a network covered by your plan, use the Find a Dentist search feature on this page. If you are not sure what network to search, log into your account to verify your coverage.
Is my dentist in-network for my plan?
You will need to do two things: Find out the type of plan in which you are enrolled (do this by logging into your account) and find out which networks your dentist participates in. To see if your dentist participates in a particular network, use the Find a Dentist search feature on this page.
What provider network do I have to use?
We offer the Delta Dental Premier and the Delta Dental PPO networks. Together, they make our Dual Network, in which nearly 9 out of 10 dentists in Colorado participate.
The network you should use is influenced by several things. Some plans allow you to see providers in either network: PPO or Premier. With many of these plans, you will receive the best benefits by seeing a PPO provider. Other plans restrict subscribers to seeing only PPO providers, so it’s important to check your specific benefits and plan type before visiting the dentist.
If you are enrolled in a Patient Direct discount plan, you must see a provider who participates in the Patient Direct network in order to receive discounts.
You can check your coverage by logging into your account. Why wasn’t my full claim paid?
Most plans require co-insurance and deductibles for certain procedures, meaning that the patient is responsible for a portion of the charges for the treatment. If you notice a claim that was not paid in full, it is most likely because the procedure was not covered at 100% under your specific insurance plan, or your deductible had not been satisfied.
Another reason you may be responsible for a portion of the charges is if you see a Premier or non-participating provider, as opposed to a PPO provider. However, this varies by plan. It is important to check your specific benefits by logging into your account.
How do I get an Explanation of Benefits (EOB)?
Your EOB will be mailed to the address we have on file. However, you may view your EOBs anytime by logging into your account using the Subscriber Login box. If you are having trouble understanding your EOB, view our handy Guide to Understanding your EOB
